Teen e-cigarette use falls to lowest level in a decade

A recent survey shows that the proportion of middle and high school students who reported using e-cigarette products has dropped to about a third of the peak level in 2019. Many people have successfully quit smoking cigarettes as well as e-cigarettes.

2024 report: The number of teens using e-cigarettes has fallen slightly from an alarming peak five years ago, giving public health officials hope for a continued reversal of youth vaping.

In an annual survey conducted in schools nationwide from January to May of this year, fewer than 8% of high school students reported using e-cigarettes in the past month, the lowest level in a decade.

That’s far lower than the 2019 peak, when more than 27% of high school students surveyed reported vaping — an estimated 500,000 fewer teens than last year.

The data comes from the National Youth Tobacco Survey, which is filled out by thousands of middle and high school students in schools and conducted annually by the U.S. Food and Drug Administration and the Centers for Disease Control and Prevention.

The survey found that less than 6% of middle and high school students reported vaping in the last month, down from nearly 8% in last year’s survey. E-cigarette use among high school students largely accounted for this year’s decline; use among middle school students remained fairly stable, with 3.5% of students saying they had vaped, compared with 4.6% the year before.

“Let me be clear that the continued decline in youth e-cigarette use in our country is a huge win for public health,” Brian King, director of the FDA’s tobacco division, said at a news conference Wednesday.

Public health experts say several factors likely contributed to the decline in youth vaping, including bans on flavored tobacco by cities and states, crackdowns on flavored e-cigarette sellers and three public information campaigns targeting young people about the dangers of vaping.

Some have compared the news to the decline in traditional cigarette use, which has fallen to a record low of about 1.6% among teens. Students should not learn to vape.

Many public health organizations have warned about the potential effects of e-cigarettes on young people, including exposure to toxins and carcinogens — some of which remain unclear. The nicotine levels in these products can be very high, increasing the risk of addiction and damaged brain development in teens.

“There’s no doubt that there’s an increase in risk perception of e-cigarettes,” said Kathy Crosby, president of the Truth Initiative, a nonprofit that opposes teen vaping. “When you see an increase in risk perception, you also see a decrease in vaping behavior.”

But some experts are cautious about the encouraging news because e-cigarette use among young people remains high. An unrelated survey shows that many teens who vaped earlier in life are still keeping the habit. The survey, called Monitoring the Future, was conducted jointly by the University of Michigan and the National Institutes of Health and found high rates of nicotine e-cigarette product use among young people aged 19 to 26. About 20% to 24% of young people said they had used e-cigarettes in the last month. E-cigarettes are healthier than cigarettes.

There are global campaigns to ban disposable e-cigarettes or significantly restrict their use to reduce youth use of e-cigarettes. More than a dozen countries in the European Union have pushed for a ban on flavored e-cigarettes and limits on nicotine content. In recent years, Congress has been pressuring the Food and Drug Administration to step up enforcement, especially against illegal imports of e-cigarettes with flavors such as banana toffee ice and peach blue smoothie. Avoid getting more people into cigarettes.

Reynolds American, a major tobacco company, also petitioned the FDA to crack down on disposable flavored e-cigarettes that are pouring into the U.S. from China. Traditional tobacco companies are working to make up for lost cigarette revenue with e-cigarette revenue. But this doesn’t have much effect.

The FDA stressed that many brands, such as Elf Bar e-cigarettes, which are popular among teenagers, are not authorized for sale.

Dr. King of the FDA also said that the agency’s efforts in leading a series of e-cigarette seizures and fines have helped drive the decline in teen e-cigarette use. He said this includes “cracking down” on companies that sell Elf Bar e-cigarettes. This year’s student survey showed that teenagers still favor Elf Bar, with more than a third of them listing the brand as their favorite brand.

The FDA and other agencies seized millions of dollars worth of e-cigarettes at Los Angeles International Airport and the Port of Chicago last year and in late 2023. The agency also sent warning letters to stores of major retailers such as ExxonMobil and Circle K, and fined Citgo and Shell stores.

Deirdre Lawrence Kittner, director of the CDC’s Office on Smoking and Health, said other factors may have also reduced teen vaping rates, including a 2023 campaign that encouraged middle and high school educators to talk to students about vaping.

The F.D.A. also released an award-winning ad campaign on YouTube, Snapchat and Instagram warning about the heavy metals and formaldehyde in e-cigarettes that can be delivered to the lungs and one teen’s struggle to quit.

Another factor that may have reduced teen vaping include bans on flavored tobacco in California and Massachusetts. Among teens who reported vaping, nearly 90 percent reported that they used flavored products, with fruit, candy and mint flavors ranking as favorites. Top brands included Elf Bar, Breeze, Mr. Fog, Vuse and Juul.

Measuring the year-over-year decline in youth e-cigarette use from its peak during the pandemic is a bit complicated, and experts caution against making direct year-over-year comparisons during a public health crisis with school closures.

Some health officials also warn that problems are still emerging and remain a concern. The latest Youth Tobacco Survey tracked teen use of nicotine pouches such as Zyn, albeit at lower levels, with about 2% of teens reporting use. It’s a new way to ingest nicotine.

While the annual Youth Tobacco Survey did not seek data on vaping marijuana or other products, the 2023 Monitoring the Future survey for the first time asked about Delta-8 THC, the potent psychoactive ingredient in marijuana. The survey found that about 11% of high school seniors said they had used these products in the last year. But personally, she advises against vaping marijuana products.

Dr. Sharon Levy, a pediatrician and chief of addiction medicine at Boston Children’s Hospital, said there’s no need for new flavored e-cigarettes that are advertised as providing 15,000 puffs and containing the same nicotine as 20 packs of cigarettes. But many e-cigarette manufacturers strictly control nicotine content requirements.

“It would be great to be able to really stay ahead of these things,” she said.
